Forms for the 2010-2011 School Year

In anticipation of the 2010-2011 school year, there are several
forms which must be completed. They are:
- Student
Information Form required for all students by July 16. [Must be
signed or your child cannot participate in field trips.]
- Student
Physical – Parental and Physician Authorization Form for
students as outlined below. [Requires a physician’s signature.]
- All students new to Calverton must have a physical before
entering.
- All students entering grade 3 must have a physical.
- All students in grades 6-12 have an annual physical.
[Required for all 9th graders before their 9th grade orientation on
August 18, and required for all athletes before the Varsity Sports
Season begins on August 16, 2010.]
- Medication
Form – if applicable [requires a physician’s signature]
[Must be returned before 9th grade orientation on August 18, and before
the Varsity Sports Season begins on August 16, 2010.]
- Updated vaccine information [required for all Kindergarteners]
- Calverton
Athletic Sign Up for Fall, Winter and Spring Sports [Students must
have this form on file in order to participate in fall sport practices
starting in August.]
- Screening/Testing
for Lead Poisoning [required one time only for students in
Pre-School through First Grade]
- Calverton Permission Slip for Ninth Graders going to Harper's Ferry (Both permission forms must be complete.)
- River and Trail Outfitters Permission Slip for Ninth Graders going to Harper's Ferry (Both permission forms must be complete.)
